Swadlincote Families Asked To Help Relatives Return Home From Hospital

Joined Up Care Derbyshire has urged families across Derby and Derbyshire — including those in Swadlincote — to help relatives leave hospital as soon as they are medically fit to go home. Health officials are asking residents to assist with simple tasks such as arranging transport, preparing the home and stocking essentials to help free up beds for other patients.

Local hospitals, including Queen’s Hospital Burton and Royal Derby Hospital, are currently facing high demand for services. According to Joined Up Care Derbyshire, helping patients return home quickly not only improves hospital flow but also supports recovery and helps maintain people’s independence.

The push for timely discharges comes while Derbyshire County Council is consulting on changes to local care facilities. Council papers and local reporting show Oaklands in Swadlincote has been identified in proposals as a candidate to operate as a specialist dementia care site or to provide community support/discharge beds (short-term rehabilitation beds), but formal decisions were part of a council consultation process.

For families who need extra support, South Derbyshire CVS offers a Home From Hospital service that helps with tasks such as collecting prescriptions, arranging travel and settling back into a routine. Residents can also use local transport services such as Swadlincote Community Transport or Derbyshire Connect if they need help getting to follow-up appointments.

From 1 January 2026, Continuing Healthcare (CHC) and Continuing Care services for Derby and Derbyshire began to be provided by the Derby and Derbyshire Integrated Care Board; the change is intended to make the move from hospital to home smoother. Health and care teams are now promoting a “Home First”/discharge-to-assess approach that focuses on providing care at home where clinically appropriate.
